Saving the Form to Your Computer
If you wish to save the form to your computer, Go to the File menu, click Save As. Double-click the folder you want to save the page in. In the File name box, type a name for the page (or use the default name).

Trouble Pasting Images Into The Form Field
Some images from Word will paste into the form field, but if you are having problems, create your page in Word, then save the document as a pdf.

Follow the instructions above for saving the form to your computer. You will also need a copy of Adobe Acrobat Professional.

Using Adobe Acrobat Professional, open the form, chose Document > Insert Pages.

Select the PDF you created from Word, and click Select.

Specify the location and the page where you want to place the inserted document, and click OK.

OR

1. Download the application in Word.

2. Complete the application, save it as a PDF file (to preserve the formatting), send the PDF version to gshepherd@parseghian.org (the PDF version does not need a signature).

3. Print a copy of the application, sign, and send via postal mail to:

Signatures
Signatures are only needed on hard copy that is sent via postal mail (see application instructions for details on hard copy). A signature is not needed on the electronic version sent via email.
